Certification Details

  • License: Active AZ or Compact license required.
  • Ensure all certifications are accurate and up to date.

Job Details

  • Start Date: ASAP (10/7)
  • Department: Pharmacy | Pharmacist
  • Emergency Medication Protocol: ACLS preferred; attendance at codes may be required.
  • Experience: Familiarity with Vancomycin AUC dosing required.
  • Systems: Proficiency with EPIC, Lexicomp, Baxter IQ essential.
  • Shifts: Day and evening shifts needed; current day availability preferred.
  • Weekend Requirements: Every other weekend or one per month preferred.
  • Holiday Requirements: 2/3 holidays required.
  • Medication Dispensing System Experience: Omnicell, Central pharmacy robot, carousel, Omnicell XT cabinets, and Omnicell Controlled Substance cabinet.
  • Dress Code: Business casual with a white coat or khaki scrubs.
  • Orientation: Includes one NEO, one clinical education/EPIC, and one day on the floor.
  • Schedule: 7 on, 7 off; 10-hour days, Thursday to Wednesday; evening shifts as needed.

Job Requirements

  • Must have 2 years of true hospital pharmacy experience in a similar-sized facility (250 census).
  • License: Active AZ or Compact license required.
  • Resume must detail duties, hospital size, unit size, and trauma level (if applicable) for each hospital.
  • Explanation of any employment gaps required upon submission.

Schedule Information

  • Work Schedule: 7 on, 7 off; 10-hour days, Thursday to Wednesday.
  • Evening shifts as needed.
  • One clinical manager on-site 5 days a week.
  • Weekend requirements: Every other weekend or one per month preferred.
  • Holiday requirements: 2/3 holidays required.

Unit-Specific Information

  • Department Orientation: One NEO, one clinical education/EPIC, one day on the floor.
  • Unit/Department: Pharmacy.

Additional Details

  • ACLS preferred; attendance at codes may be required.
  • Preference for IV pharmacist/compounding pharmacist.
  • EPIC experience required with no inconsistencies between resume and cover letter.
  • EMR experience must be listed on each work assignment.
  • Time off requests limited to 5 days or less and should be included within the attached profile/resume.
