Job Overview:

Become an essential part of patient care as a Clinical Pharmacist at McCullough Hyde Memorial Hospital! We are looking for a skilled and dedicated Clinical Pharmacist to join our multidisciplinary team. In this role, you will collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and individualized medication therapy for patients, directly impacting patient outcomes.

Key Responsibilities:

  • Collaborative Care: Work closely with healthcare professionals to optimize medication therapy and improve patient outcomes.
  • Patient Education & Counseling: Provide critical medication therapy management and counseling to patients regarding their drug therapies.
  • Medication Safety: Ensure the 7 patient rights (right patient, right drug, right dose, right time, right route, right information, right documentation).
  • Pharmaceutical Expertise: Contribute to drug therapy management, medication reconciliation, and clinical care through detailed medication review and assessment.
  • Regulatory Compliance: Ensure compliance with organizational policies and regulatory requirements.
  • Supervision & Training: Lead and supervise team members to maintain efficiency and accuracy in medication distribution. Precept pharmacy students and residents, contributing to the development of future pharmacy professionals.
  • Process Improvement: Lead pharmacy process improvements, identify opportunities, and implement solutions to enhance patient care and medication safety.

Minimum Qualifications:

  • Education: Bachelor’s Degree in Pharmacy (PharmD preferred).
  • Licensure: Active pharmacist license in the state of Ohio.
  • Experience: 1-2 years of experience as a Pharmacist. Inpatient or infusion pharmacy experience preferred.
  • Skills: Strong interpersonal, analytical, and problem-solving skills.

Working Conditions:

  • Hours: Tuesday and Wednesday from 7:30 AM - 6:00 PM; every 3rd weekend and holiday required.
  • Physical Requirements: Consistent reading, thinking, reasoning, and communication. Frequent walking and standing; occasional lifting of up to 50 lbs.
