Role Overview

The Patient Care Coordinator plays a key role in supporting patients throughout their pharmacy care journey. This role focuses on patient communication, care coordination, adherence support, and collaboration with healthcare providers and internal pharmacy teams to ensure timely and effective therapy management.


Work Schedule & Compensation

  • Shift: Monday – Friday, 8-hour shifts (8:00 AM – 8:00 PM EST, based on operational needs)

  • Work Mode: Remote (must reside within 50 miles of 6001 Global Distribution Way #102, Louisville, KY 40228)

  • Pay Rate: $20.00/hour


Key Responsibilities

1. Patient Support & Communication

  • Respond to patient inquiries within defined timelines

  • Provide guidance on drug therapy and pharmacy care concerns

  • Support patients facing physical, psychological, and social challenges related to disease states

2. Medication Adherence & Care Support

  • Promote medication adherence and compliance

  • Assist patients in understanding their treatment plans

  • Participate in the development and administration of pharmacy care plans

3. Clinical Coordination & Reporting

  • Report adverse drug events (ADEs) and therapy-related issues

  • Maintain accurate and up-to-date documentation in patient records

  • Flag critical issues for pharmacist or clinical review

4. Care Team Collaboration

  • Work closely with physicians, pharmacists, nurses, payers, labs, and other healthcare professionals

  • Participate in multidisciplinary discussions regarding patient care plans

  • Coordinate communication between internal and external stakeholders

5. Issue Resolution & Case Management

  • Troubleshoot patient and payer-related issues efficiently

  • Coordinate resolutions with appropriate teams

  • Ensure timely escalation when necessary

6. Quality Improvement & Process Support

  • Identify opportunities for improving patient service and workflow efficiency

  • Contribute to continuous quality improvement initiatives

  • Share feedback with healthcare and pharmacy teams


Required Qualifications

Education & Experience

  • High school diploma required

  • Minimum 2 years of experience in:

    • Healthcare call center, OR

    • Physician office, OR

    • Social services environment

  • Preferred: Associate degree in Business Administration, Marketing, Sales, IT, or related field (or equivalent experience)
