Job Overview
We are seeking a qualified Clinical Chart Reviewer (CCR) to join a leading US healthcare clinical services organization. The role involves reviewing patient medical records, identifying potential conditions, and contributing to value-based care initiatives by analyzing comprehensive patient data.
The ideal candidate will have a strong understanding of clinical evaluation, risk assessment, and patient care gaps in the US healthcare context.
Key Responsibilities
Review longitudinal patient medical records and provide clinical opinions on potential conditions based on symptoms, medications, lab results, clinical notes, social and family history, and other relevant data.
Understand US value-based healthcare, including patient risk factors, chronic conditions, comorbidities, and factors driving healthcare utilization.
Conduct thorough evaluation of patient documents to identify missed diagnoses, gaps in care, and other evidence of risks or disease progression.
Ensure accurate and consistent documentation of findings to support clinical decision-making and patient care optimization.
Collaborate with clinical teams to standardize and scale review processes and services for clients.
Required Skills & Competencies
MBBS qualification with a clinical understanding of patient evaluation, disease patterns, and risk assessment.
Knowledge of US healthcare practices, chronic disease management, and value-based care concepts is preferred.
Strong analytical and observational skills to identify gaps in care and potential risks.
Excellent written and verbal communication skills.
Ability to work in a fast-paced office environment, adhering to timelines and quality standards.
