Schedule:

  • Monday to Friday: 7:00 AM - 3:00 PM; 10:30 AM - 6:30 PM; 11:30 AM - 7:30 PM (Rotating Shifts)
  • Rotating Weekends: Saturday 8:00 AM - 4:00 PM & Sunday 8:00 AM - 2:00 PM

Responsibilities:

  • Evaluate Medication Orders: Assess physician orders for appropriateness, dosage, potential interactions, and route of administration. Make therapy recommendations when necessary.
  • Patient/Resident Database Management: Maintain current and historical medical data, clinical information, and medications.
  • Monitor Lab Tests: Ensure that necessary lab tests for high-risk conditions are ordered, evaluated, and discussed with the healthcare team.
  • Collaboration: Work with the healthcare team to provide clinical and technical support as needed.
  • Documentation: Document all communications with healthcare staff, physicians, patients, and their families in the medical record.
  • Medication Safety: Implement policies for safe procurement, storage, distribution, and disposal of drugs. Report any medication errors to the Pharmacy Manager.
  • Assist in Procedures: Support facility staff with safe methods of procuring, maintaining, administering, and disposing of medications.

Qualifications:

  • Education: Bachelor’s Degree in Pharmacy (PharmD preferred)
  • Experience: One year of experience in long-term care is preferred.
  • Licensure/Certifications: As required by government regulations and business needs.
  • Skills: Detail-oriented, goal-driven, team player, and strong communication skills.
