Key Responsibilities

Patient Care

  • Conduct drug utilization reviews, patient counseling, and medication therapy management.

  • Manage collaborative drug therapy under valid agreements.

  • Prescribe medications and devices as per protocols.

  • Order, interpret, and monitor laboratory tests.

Pharmacy Oversight

  • Supervise pharmacy personnel, ensuring compliance with licensure scopes.

  • Safeguard the pharmacy area and prevent theft/diversion of drugs.

  • Maintain records and inventories in line with state and federal regulations.

  • Dispense prescriptions accurately according to provider authorization and policies.

Professional Development

  • Maintain active licensure and pharmacological knowledge through workshops, publications, and professional networks.

Compliance

  • Perform clinical duties in alignment with The Joint Commission (TJC) and Health Resources and Services Administration (HRSA) standards.

Other Duties

  • Participate in additional responsibilities as assigned.


Qualifications

  • Education:

    • Bachelor's Degree in Pharmacy required. PharmD preferred.

  • Experience:

    • Experience in community or ambulatory pharmacy practice preferred.

    • PGY-1 Residency equals 2 years of pharmacist experience; PGY-2 equals 3 years.

  • Licensure:

    • Current OR State Pharmacist License.

  • Skills:

    • Expertise in drug therapy and community pharmacy practices.

    • Knowledge of state and federal pharmacy laws.

    • Proficiency with pharmacy software and Microsoft Office.

    • Strong attention to detail and communication skills.

    • Ability to prioritize tasks in a fast-paced environment.
