Job Summary
We are looking for a Senior Medical Coder (HCC – Risk Adjustment Coding) with 5–6 years of experience in reviewing medical records and abstracting HCC codes as per industry standards. This role is ideal for professionals with strong expertise in ICD-9-CM coding, medical terminology, and chart review guidelines, aiming to deliver high-quality coding services while meeting productivity and accuracy targets.
Responsibilities
Perform medical record reviews (provider reports, inpatient and outpatient records) to identify and assign HCC (Part C & Part D) diagnoses.
Abstract accurate diagnosis codes from provider documentation and approved documentation sources.
Ensure coding aligns with official guidelines and client-specific requirements.
Achieve and maintain production and quality standards set by management.
Address and resolve coding queries promptly through effective communication with the team and reporting managers.
Complete administrative tasks and maintain compliance with company protocols.
Required Skills & Qualifications
Graduate in Life Sciences or related healthcare discipline.
5–6 years of experience in HCC (Risk Adjustment) coding and chart review.
Expertise in ICD-9-CM coding guidelines and HCC-specific guidelines.
AAPC or AHIMA certification is mandatory.
Strong knowledge of medical terminology, anatomy, and disease conditions.
Familiarity with HIPAA compliance standards and data security protocols.
