Job Overview

UCLA Health is seeking a Transplant Pharmacist to deliver advanced pharmaceutical care for transplant patients.

The role focuses on providing both pre- and post-transplant clinical services, optimizing drug therapy, and ensuring safe and effective medication use. The pharmacist will work closely with multidisciplinary teams and contribute to patient care, education, and quality improvement initiatives.

Key Responsibilities

  1. Clinical Pharmacy Services

Provide advanced pharmaceutical care to transplant recipients.

Support pre- and post-transplant medication management.

Ensure safe, effective, and evidence-based drug therapy.

  1. Therapeutic Optimization

Develop and optimize drug protocols for transplant patients.

Monitor therapy outcomes and adjust treatment plans accordingly.

Ensure compliance with clinical guidelines and best practices.

  1. Patient Care & Support

Collaborate with healthcare teams to deliver comprehensive care.

Provide medication counseling and patient education.

Support continuity of care throughout transplant journey.

  1. Collaboration & Multidisciplinary Work

Work closely with physicians, nurses, and transplant teams.

Participate in interdisciplinary care planning and decision-making.

  1. Education & Quality Improvement

Contribute to staff education and training programs.

Participate in quality improvement initiatives.

Support clinical committees and institutional programs.

  1. Operational Responsibilities

Work varied shifts including weekends and holidays.

Ensure adherence to hospital policies and regulatory standards.

Required Skills

Clinical Expertise

Strong knowledge of transplant pharmacotherapy.

Experience in managing complex medication regimens.

Understanding of immunosuppressive therapies.

Communication & Teamwork
